Position Duties: This position is responsible for providing direct supervision to providers assigned to Home and Community Based Waiver Services, (HCBW) and case management services to individuals experiencing development disabilities. This includes but is not limited to consumer assessment of equipment and home needs, habilitation goals, goal development, and reviews. Coordinates services with other agencies to advocate on the consumers behalf to achieve needed services. Responsible for a caseload of up to 25 and assisting with monthly statistical information and information gathering as assigned. The Medicaid Supervisor/Case Manager will ensure quality services are being provided by working with the provider and consumer in the home or out in the community, conduct regular goal reviews, provide emotional support, assist in connecting consumers with available community resources, and develop and maintain interagency relationships.
Minimum Qualifications: Personal: Must maintain program/consumer/agency confidentiality; treat each individual with dignity and respect. Have good people skills; self-starter; ability to accept supervision; and enjoy working with co-workers, consumers, parents/guardians, and other community professionals. Ability to set and observe personal limits and boundaries. Demonstrated leadership, maturity, flexibility, discretion, dependability and motivation. Ability and desire to work with a variety of people to carry out the mission and purpose of the program. Ability to be a team member with program staff, agency personnel, and community service providers. Ability to cope with emergency situations, (i.e., provide emergency services, accidents, seizures, and etc.) in a calm and efficient manner
Education/Experience: Bachelor’s in Special Education, Human Services, Social Work, Family Counseling, Psychology, Public Health, or related field preferred. Previous DD and supervisory experience required; Medicaid waiver experience helpful.
